Welcome to Alleviate Pain and Wellness Where Relief Starts with Understanding

Becoming a new patient is an important step, especially when you’ve been living with chronic pain. Our goal is to make the process simple, supportive, and transparent, so you know exactly what to expect before your first visit.

Below is everything you need to get started with Mary Colburn, APRN, FNP-C, including referral requirements, what to bring, and how to prepare for your appointment.

  • To schedule as a new patient, we require a referral from your healthcare provider. This helps ensure we have the clinical information needed to safely and appropriately manage your chronic pain.

Please ask your provider to fax the following to 203-626-1624:

    • Recent office notes related to your chronic pain

    • Relevant imaging (X-ray, MRI, CT or ultrasound)

    • Pertinent labs

Once your documents are received and reviewed, our office will contact you to schedule your appointment.

  • Your first appointment is a comprehensive evaluation designed to help Mary understand your pain story and the goals you have for relief and daily function.

    Your visit may include:

    • A discussion of your symptoms and where the pain impacts your life

    • Review of medications and past treatments

    • Going over imaging and lab results

    • Understanding triggers, patterns, and past challenges

    • Setting shared goals for your treatment plan

Mary takes the time to listen, your experiences, frustrations, and concerns matter here.
