Becoming a new patient is an important step, especially when you’ve been living with chronic pain. Our goal is to make the process simple, supportive, and transparent, so you know exactly what to expect before your first visit.
Below is everything you need to get started with Mary Colburn, APRN, FNP-C, including referral requirements, what to bring, and how to prepare for your appointment.

To schedule as a new patient, we require a referral from your healthcare provider. This helps ensure we have the clinical information needed to safely and appropriately manage your chronic pain.
Please ask your provider to fax the following to 203-626-1624:
Recent office notes related to your chronic pain
Relevant imaging (X-ray, MRI, CT or ultrasound)
Pertinent labs
Once your documents are received and reviewed, our office will contact you to schedule your appointment.

Your first appointment is a comprehensive evaluation designed to help Mary understand your pain story and the goals you have for relief and daily function.
Your visit may include:
A discussion of your symptoms and where the pain impacts your life
Review of medications and past treatments
Going over imaging and lab results
Understanding triggers, patterns, and past challenges
Setting shared goals for your treatment plan
Mary takes the time to listen, your experiences, frustrations, and concerns matter here.

To ensure the most accurate and personalized care, please bring:
A list of your current medications
Any recent imaging reports (if not already faxed)
Insurance card and photo ID
A list of past treatments that have or have not helped
Questions you’d like to discuss with Mary
Many patients find it helpful to write down symptoms or concerns ahead of time.

To make your first visit as smooth and productive as possible:
Arrive 10–15 minutes early to complete any remaining paperwork
Bring any mobility devices, braces, or assistive tools you use regularly
Wear comfortable clothing that allows gentle movement
Be prepared to discuss how pain affects your daily activities
Our space is designed to be calm, supportive, and patient-friendly.

Following your evaluation, Mary will create a personalized treatment plan which may include:
Non-opioid or opioid medication options (when appropriate)
Trigger point injections
Suboxone treatment or tapering support
Follow-up visits to monitor progress and adjust care
Communication with your primary care provider or specialists
You will always know the next steps, what to expect, and how to reach us if questions arise.
